چکیده

A new method of preventing recurrent dislocation following total hip replacement was described by Olerud and Karlstr#{246}m (1985). It involves augmentation of the acetabular component with a sector cut from another acetabular prosthesis and screwed to the original implant. This procedure was performed on three of our patients with recurrent dislocation, but in each case the hip continued to dislocate after the operation, despite the fact that all three were performed (as recommended in the original paper) by experienced surgeons. In each case the failure was due to impingement of the neck of the femoral component on the augmentation. The added sector acted as a fulcrum for the femoral neck, causing the femoral head to be levered out of the acetabulum in the opposite direction to the original dislocation (Fig. 1).

Bilateral Staged Total Hip Replacement and the Natural Progress of an Untreated Case of Developmental Dysplasia (Dislocation) of the Hip: A Clinical Case Report by the Surgeon and the Patient

The natural history of an untreated case of a Developmental Dysplasia (Dislocation) of the Hip (DDH) associated with multiple congenital abnormalities is reported in a 55-years-old man. The patient’s complaints and the varieties of the typical manifestations emerged in other parts of the body throughout the life are reviewed and discussed as comorbidities of a dysplastic condition. Two-stage bi...

Flexion reminder device to discourage recurrent posterior dislocation of a total hip replacement: a case report

INTRODUCTION Recurrent dislocation of a total hip replacement prosthesis is a frustrating complication for both the surgeon and the patient. For positional dislocations with no indications for revision surgery, the current best treatment is physiotherapy, the use of abduction braces and avoidance of unsafe hip positions. Abduction braces can be cumbersome and have poor compliance. We report the...

Cup re-enforcement for recurrent dislocation after hip replacement.

Recurrent dislocation after total hip replacement usually demands revision. Ahnfelt (1986) reported 626 disbocations of which 79 (1 1.2%) dislocated twice, 33 (5.1%) three times and 40 dislocated (6.6%) four times. We now report the results of treating 1 3 cases of recurrent dislocation by fixing an additional sector to the posterior rim ofthe acetabular component after Charnley total hip repla...

Reconstruction of the Acetabulum in Developmental Dysplasia of the Hip in Total Hip Replacement

Developmental dysplasia of the hip (DDH) or congenital hip dysplasia (CDH) is the most prevalent developmental childhood hip disorder. It includes a wide spectrum of hip abnormalities ranging from dysplasia to subluxation and complete dislocation of the hip joint. The natural history of neglected DDH in adults is highly variable. The mean age of onset of symptoms is 34.5 years for dysplastic DD...

Dislocation after primary arthroplasty for subcapital fracture of the hip. Wide range of movement is a risk factor.

Dislocation is the most frequent serious complication following total hip replacement for subcapital femoral fracture. We report a prospective study, using matched groups, which compared the range of hip movement following hip replacement for arthritis and for fracture. The range of movement was significantly greater in the fracture group. We suggest that this is a predisposing factor for dislo...
